Palliative Care (PC) programme was established at Nyakahanga DDH (NDDH) in 2008 and incorporated these care within hospital services. NDDH is the only hospital that provides PC in the area – serving as a referral centre for 70 health facilities in Karagwe and Kyerwa districts. Our PC programme is clocking 13 years since inception, providing both facility-based and community-based PC. PROGRAMME VISION Everyone living in Karagwe community and beyond with a life – limiting illness will have to access to quality palliative care that is delivered in affordable and cultural oriented manner. PROGRAMME MISSION Nyakahanga Palliative care programme is committed to provision of holistic care to people living with life threatening conditions through advocacy, education and passionate palliative care in a way that proclaims the Gospel. OUR CORE VALUES Love, team-spirit, voluntarism, respect, sharing, recognition of talents, and accountability to each other PROGRAMME ACTIVITIES In-patient and outpatient care provision Home visits conducted regularly with community volunteer workers (CVWs) with back-up of our core team whenever needed. Day care clinics (outreach services) done once a week in different wards/ congregations Referral to other centres – higher health facilities for further and expertise management Monthly meeting with community volunteer workers Community sensitization and local resource mobilization Advocacy
